Seaman & Co., Inc. will provide enrolled employers with an IRS
compliant POP Plan that allows eligible, enrolled employees and dependents to
pre-tax payroll deductions for premium contributions.

PPO DENTAL PLANS REQUIRE A MINIMUM
OF 5 EMPLOYEES FROM A GROUP TO ENROLL AS MEMBERS.
