CITY CARE BENEFITS TRUST PLAN

The Business Improvement District Council (BIDC), a non-profit organization, provides services to employers with a minimum of 2 employees in the City of San Diego.  In an effort to provide affordable health insurance coverage to employers, they have developed an ERISA Trust Plan, CITY CARE BENEFITS, working with their consultant, Phillip D. Nico; and to be offered to employers with 2 or more eligible, full-time employees and have a City of San Diego Business License.

 

There are more than 70,000, City of San Diego, licensed businesses throughout the San Diego area.  This plan is only available through a California licensed health insurance agent or broker.  Employers who do not have an agent or broker will be referred to a “Preferred Agent or Broker”.

 

·        Sharp Health Plan was selected to underwrite the Medical coverage.

·        Golden West was selected to underwrite the Dental and the Voluntary Dental & Vision coverage.

·        Safeguard was selected to underwrite the employer provided Vision coverage.

·        American Specialty Health Plan was selected to underwrite the Chiropractic / Chiropractic & Acupuncture coverage.

·        The Hartford was selected to underwrite the Term Life coverage.

·        Seaman & Co., Inc. was selected to provide all broker/agent marketing, enrollment and premium administration services.

·        Composite rates (no age-rates or rate adjustment factor).

·        No health questions.

·        Guaranteed issue for employers with 2 or more eligible, full-time employees working 30 hours or more per week (5 employees must enroll for PPO dental).

·        Eligible dependents are the employee’s legally married spouse, domestic partner (affidavit required) and unmarried dependent children to age 19; and to age 25 if full-time students (certification required).  Domestic Partners are not eligible for Dependent Life coverage.

·        50% employer contribution of the Employee Only premium.

·        100% contribution of the premium for the Employee and Dependent Term Life plans.

·        70% of eligible employees must participate in the Medical plan; 75% of eligible employees must participate in the Dental and/or Vision plans.

·        The Medical, Dental and Vision Plans are not packaged.  The employer can have a Medical plan only, a Dental plan only or a Vision plan only or a combination of two or three plans.

·        Sharp HMO Medical can be dual-choice with Kaiser.

·        Both the high and the low option Sharp HMO plans can be offered to the same employer group.

·        Sharp Health Plan will no longer allow a rollover Sharp HMO group into City Care Benefits Trust Plan until a period of 12 months after the date of termination has elapsed.  Groups rolling over from California Choice or Pac Advantage are not subject to this provision and can roll over at any time

·        Partnerships must be notarized.

·        An IRS compliant Section 125 Premium Only Plan (POP), that allows eligible, enrolled employees to pre-tax payroll deductions for premium contributions, will be provided at no cost.

·        Employer must have a City of San Diego Business License.
