4.   Providers.  Sharp Health Plan’s physician and hospital network and the other health plan providers are located throughout the county of San Diego and southern Riverside County to provide all covered members with an exceptional selection of medical providers.  The Golden West, Safeguard and American Specialty Health Plan providers are located throughout the state of California.

 

5.   Service.  24-hour Membership Services is provided by all the carriers.

 

6.   Plan Selection.  You may want to offer only one plan; i.e., Sharp HMO, Golden West Dental, Safeguard Vision or American Specialty Health Plan’s Chiropractic & Acupuncture, or any combination of these plans to your employees.

 

7.   Administrative Services.  We have selected Seaman & Co., Inc., a San Diego company, to provide all broker/agent, enrollment and premium administration services for the City Care Benefits Trust.

 

8.   Free Section 125 Premium Only Plan (POP).  Seaman & Co., Inc. will provide enrolled employers with an IRS compliant POP Plan that allows eligible, enrolled employees and dependents to pre-tax payroll deductions for premium contributions.

SHARP HEALTH PLAN HMO:

 

·        NO HEALTH QUESTIONS.

·        NO RATE ADJUSTMENT FACTOR (RAF).

·        A MINIMUM OF 2 ELIGIBLE FULL-TIME EMPLOYEES IS REQUIRED.

·        CHOICE OF HIGH OR LOW OPTION OR L-11 PLANS

·        INFORCE SHARP HMO GROUPS CAN NOT RE-ENROLL IN CITY CARE BENEFITS AT RENEWAL. 

(Groups must not have been previously enrolled in a Sharp plan for a period of 12 months).

·        RATES ARE GUARANTEED TO AUGUST 1, 2007.

 

GOLDEN WEST PPO DENTAL BENEFITS*:

 

·        BOTH PPO DENTAL PLANS COVER MAJOR DENTAL SERVICES FROM DAY 1.  NO WAITING PERIODS FOR ANY DENTAL SERVICES FOR NEWLY ELIGIBLE EMPLOYEES AND DEPENDENTS.

(There is a 12 month waiting period for late enrollees.)

·        PPO DENTAL PLANS REQUIRE A MINIMUM OF 5 EMPLOYEES FROM A GROUP TO ENROLL AS MEMBERS.

·        RATES ARE GUARANTEED TO AUGUST 1, 2007.

 

GOLDEN WEST HMO DENTAL BENEFITS*:

 

·        HMO DENTAL PLANS REQUIRE A MINIMUM OF 2 ELIGIBLE EMPLOYEES.

·        HMO DENTAL PLANS INCLUDE COSMETIC DENTAL COVERAGE, ORTHODONTIA & VISION COVERAGE.

·        RATES ARE GUARANTEED TO AUGUST 1, 2007  .

 

GOLDEN WEST & SAFEGUARDS VISION BENEFITS*:

 

·        BOTH PPO AND MANAGED VISION PLANS REQUIRE A MINIMUM OF 2 OR MORE ELIGIBLE EMPLOYEES IN A GROUP.

·        RATES ARE GUARANTEED TO AUGUST 1, 2007.

 

*GOLDEN WEST REQUIRES THAT ALL SELECTED PLANS MUST BE HIGH OR LOW OPTIONS.  DUAL CHOICE IS AVAILABLE ON DENTAL COVERAGE. 

 

MONTHLY ADMINISTRATION FEE:  $10.00 PER MONTH (PER GROUP).
